Can't vouch for them, but cheapnhljerseys.com is probably what it sounds like -- but $35 for a named/numbered jersey (guessing it would be press-on or printed) is, well, cheap.

There are some fairly decent deals to be had on eBay. I usually go for "Buy Now" prices (like the one above) but you could also try for some auctions and get lucky.

I don't know. Never bought anything but the blanks. Regular price for the jersey is $130 and they're probably around $100 now, so that's $75 for the name/numbers. Sounds too much for heat-press, so I'd guess sewn-on.
I have to say, though, I'm not a big fan of the replicas (or "pro" as they sometimes call them). The "authentic" ones are much thicker fabric. Of course those ones go for $300, but usually at the start or end of the year they will have a sale at the Sensations stores in SBP where you can pick practice and some game-worn jerseys (pre-season, so you won't find any "stars") for $50. One year I got Emery's practice jersey (just the number, no name plate) -- didn't realize it was his until I tried it on and saw that it was a goalie cut; one of these days I'll finally have it taken in so I can wear it without the elbows sagging down to my knees. Now *that* is a nice, heavy jersey.
By the way, there are a few used jerseys on eBay with the older (2D) logo. Saw one away and two home. If I could find one of those -- in the heavier mesh -- then I'd be in heaven. And back then they made them from heavier fabric, I think -- not quite "authentic" but better than the crap they overcharge for now.
