QOs don't prevent offer sheets; they ensure that the team making the offer sheet has to pay the stipulated recompense if they do get the player -- i.e., a 2nd if you sign the guy to this much money, a first if you sign him to that much money, and so on. If a player signs a QO, that's it -- that's his deal / new contract.
At least that's how it worked under the old CBA.
